In 1987, the HealthReach family welcomed Western Maine Family Health Center. In 2011 the practice served 2,671 patients. The variety of services we provide ensures that all patients receive accessible quality care. We offer primary care and preventive services including routine physicals, chronic disease management, acute visits, minor surgery, and referrals.

Our providers collaborate with a team of physicians at Franklin Memorial Hospital in Farmington, where hospital care for our patients is provided. We also work with other state and community partners to provide on-site access to programs including Breast and Cervical Health, Employee Assistance, Nicotine Replacement Therapy, and Raising Readers.

Our health center and our staff are committed to the community, playing a vital role in supporting and aiding our neighbors through quality health care. The towns served by the health center include Canton, Fayette, Hartford, Jay, Livermore, Livermore Falls, and Sumner. In addition, patients from surrounding towns such as Chesterville, Farmington, New Sharon, Wilton and others are regularly welcomed.

The Androscoggin River, once busy with log drives, flows through Livermore Falls. Memories of the railroad and paper industry pervade the community, creating a historically compelling area. Only 12 miles south of Farmington, and 25 miles northwest of Augusta, this rural community offers year-round activities for the whole family. Snowmobiling, boating, and gorgeous views entice travelers and residents alike.
